• Premiums can be paid on a monthly or yearly basis (yearly rates= $29.80 x 12 months)

Eligibility

• Elite Benefits/TCIM Clients between the ages of 18 and 55
• Coverage terminates when the insured reaches age 65
• Coverage is subject to pre-existing conditions, exclusions, limitations and survival periods
