(1) the coverage selected must be the "same" for all employees or "classes" of employees;
(2) minimum enrolment - three (3) contracts for health & dental benefits; and
(3) 100% of the total group must participate in the health & dental portion of the plan.
Effective Date of Coverage:
(1) Coverage starts the date the employee is added to the group;
Termination of Coverage:
(1) Coverage terminates the date the group removes the employee from the contract.
Individual Certificates and Brochures:
(1) GMS will issue employees individual I.D. cards listing: contract numbers, employee's name, dependents covered by the plan, dates of birth; and
(2) GMS will provide individual employee booklets outlining plan benefits. The group administrator receives the contract wording.
Rate Guarantee:
(1) Once rates are set for the group, they are guaranteed for a minimum of 12 months.
